3. Eligibility and Account Registration

You must be at least 18 years of age to use our Services or submit information through our Site. By using our Services, you represent and warrant that you have the legal capacity to enter into these Terms.

4. Scheduling and Consultations

Our online scheduling and consultation request forms are provided for your convenience. Submitting a request does not create a binding care agreement. All service engagements require a formal care assessment, signed agreement, and verification of insurance or payment arrangements.

We reserve the right to decline service requests outside our geographic service area, scope of practice, or capacity. We will communicate any limitations clearly and, when possible, provide referrals to appropriate alternative providers.
